In an effort at assisting parents  and teachers with decisions regarding COUNSELING or ASSESSMENTS, our office has developed a series of useful questionnaires to assist in the  process. Note: These checklists are not intended to be diagnostic in nature, but are designed to provide initial  guidance regarding the need for these services.

The first checklist is designed to assist  parents with their question: SHOULD MY CHILD BE EVALUATED?

The second checklist is designed to assist parents with decisions regarding COUNSELING for their children, family or themselves.

The third checklist is designed to assist teachers or other educational personnel regarding the need for student ASSESSMENTS.

The fourth checklist is designed to assist  parents with decisions regarding PSYCHOLOGICAL, EDUCATIONAL, DEVELOPMENTAL or BEHAVIORAL ASSESSMENTS for their children.
