OFFICE POLICIES:

 

Please be aware of the following:

 

1. Payment for services is required at the time of service.  Payments for some evaluations may follow specific schedules which are reviewed when the evaluation is scheduled.

2. In general, it is the client's responsibility to submit claims to their insurance carrier for possible reimbursement. A receipt will be provided at the conclusion of each session. Note that we are currently an out of network provider for most insurance carriers. In order to ascertain your out of network benefits, contact your insurance carrier and provide our NPI number: 1619152212.

3. Counseling sessions may vary in length from 25 minutes to 50 minutes, with fees adjusted by session length. Punctuality for all appointments is vital.

 

4. We request at least 24 to 48 hour notification for counseling cancellations.

 

5. Comprehensive assessments are scheduled at least 2-3 weeks in advance will require at least one weeks notice prior to cancellation. Ongoing patterns of missed appointments will be discussed, and may incur a fee.

 

6. Confidentiality as per HIPPA guidelines is maintained.  Confidentiality issues are discussed at our initial session.

7. School visitations or attendance at school conferences may be scheduled.  These visits incur a separate fee, unless part of a comprehensive psychological/developmental/ educational assessment*.

 

8. Psychological and educational assessments completed at this office are, by law, accepted for consideration by public-school personnel, particularly when special-education services or other accommodations are sought (for example under Chapter 504).  Note:  private psychological/educational assessments submitted to the public schools must be completed by licensed school psychologists or psychologists. All evaluations completed at this office follow public school guidelines, particularly when identifying special needs children.

 

*School visits scheduled over three months subsequent to the preparation of the comprehensive report will however incur a fee (unless the delay is outside the parents control).  Comprehensive evaluations, as discussed, generally include intellectual, achievement, and information processing, developmental and behavioral assessments.

OFFICE HOURS:

Monday through Thursday, from 8:30 AM until 6 PM. Friday morning appointments are scheduled as needed.  School visits are typically arranged during the earlier part of the school day, if possible.
