PARENT and TEACHER SCREENING CHECKLISTS

In an effort at assisting parents  and teachers with decisions regarding  COUNSELING or ASSESSMENTS, our office has developed a series of useful questionnaires to assist in the  process. If, after reviewing these checklists, further questions remain or followup is contemplated,please contact our office ( drstu525@cs.com) - (954) 721-5144.Note:These checklists are not intended to be diagnostic in nature, but are designed to provide initial  guidance regarding the need for these services.

The first checklist is designed to assist  parents with decisions regarding PSYCHOLOGICAL-EDUCATIONAL-DEVELOPMENTAL or BEHAVIORAL ASSESSMENTS for their children.

The second checklist is designed to assist parents with decisions regarding COUNSELING for their children, family or themselves.

The third checklist is designed to assist teachers or other educational personnel regarding the need for student ASSESSMENTS.
